Effective Date
This Notice is effective as of July 21, 2026 and describes how protected health information ("PHI") related to your care through Soluna Specialty Rx may be used and disclosed, and how you can access that information.
Who This Notice Covers
Your protected health information collected in connection with a Soluna Specialty Rx evaluation — intake responses, prescriptions, visit history, and provider notes — is created, stored, and processed within PracticeQ, a HIPAA-covered clinical system used by the licensed, independent healthcare providers who evaluate your request. It is never stored in, or accessible from, your general Soluna account, this website, or any Soluna app — see our Privacy Policy for how that separate, non-clinical account information is handled.
Our Duty to Protect Your Health Information
We are required by law to maintain the privacy of your PHI, to provide you with this Notice describing our legal duties and privacy practices, and to follow the terms currently in effect.
How We May Use and Disclose Your Health Information
- For Treatment. To evaluate your request, make prescribing decisions, and coordinate with the compounding pharmacy that fills an approved prescription.
- For Payment. To process payment for services, including verifying coverage or processing your payment method, and related billing activities.
- For Healthcare Operations. For quality review, provider training, and other activities necessary to support and improve the clinical care you receive.
- As Required by Law. When federal, state, or local law requires disclosure, including in response to valid legal process.
- Public Health and Safety. To public health authorities, to report suspected abuse or neglect where required, or to prevent a serious threat to health or safety.
- Health Oversight and Legal Proceedings. To health oversight agencies for activities authorized by law, or in response to a court order, subpoena, or similar legal process.
- With Your Written Authorization. For any use or disclosure not described above, we will ask for your written authorization, which you may revoke at any time.
We do not use or disclose your PHI for marketing purposes, and we do not sell your PHI.
Your Rights Regarding Your Health Information
- Right to Access. You may request a copy of your PHI held in that clinical system.
- Right to Amend. You may request that we correct PHI you believe is incorrect or incomplete.
- Right to an Accounting of Disclosures. You may request a list of certain disclosures we've made of your PHI.
- Right to Request Restrictions. You may ask us to limit how we use or disclose your PHI; we are not always required to agree.
- Right to Confidential Communications. You may request that we communicate with you about your care in a specific way or at a specific location.
- Right to a Paper Copy. You may request a paper copy of this Notice at any time, even if you agreed to receive it electronically.
- Right to File a Complaint. You may file a complaint with us or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated. We will not retaliate against you for filing a complaint.

Changes to This Notice
We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have as well as PHI we receive in the future. The current version will always be available on this page.
